- Calling the carrier to obtain the claim number, adjuster information, mailing or fax information and other necessary data.
- Obtaining the UB04 or Form 1500 and all applicable medical records form the client
- Appropriate documentation of claim number on all records and having those records delivered to the specific adjustor
- Following-up with the carrier if the claim has not been paid within 30 days – for all claims
- If no first report of injury is on file with the specific carrier, provide appropriate forms to employer and provide specific instructions, including telephone numbers, to successfully file a first report of injury
- If the employer is uncooperative, we provide the patient with the information to file his/her own first report of injury
- Provide itemized statements, UB04, Form 1500 and medical records to all parties needed under specific state statutes
- Continue follow-up process until final resolution is established
- After payment is received, the claim is reviewed for underpayments due to PPO adjustments, improper coding or other relevant underpayment anomalies
